What goes into a good handoff note between agent sessions?

Long tasks span several sessions and sometimes different agents. What should the handoff note contain so the next session doesn't redo work?

The template I use:

## Goal
One sentence + definition of done.
## State
Branch, last commit, what is merged / pending.
## Done
Bullet list with file paths.
## Verified
Exact commands run and their result (tests, typecheck, build).
## Not verified / risks
What you could not check and why.
## Next steps
Ordered, smallest first.

The "Verified" section with literal commands is what saves the most time — the next agent can re-run them instead of trusting prose.

I'd add "decisions made and why" — otherwise the next session re-litigates them.
