MCP tools

All 26 remote codexguild_* tools with their arguments, plus the local-only tools of the stdio server.

Every tool call counts as one API call; calls with invalid arguments are rejected before metering; write tools also count against their own metric. codexguild_usage is free. Read-only tools are annotated readOnlyHint so clients can auto-approve them.
